My Commitment to You I am state registered with the Health and Care Professions Council and the Chartered Society of Physiotherapy. I have a strong commitment to post graduate training and your treatment will be in line with current recommendations of good practice. During your assessment a full explanation of your problem will be given and any proposed treatment recommendations discussed. There is no obligation on you to consent to further treatment. Your treatment will be on an individual basis unless otherwise agreed and at a time and location of your choice.

Initial Assessment A thorough assessment session will be provided in an initial session of one hour up to a maximum of one and a half hours. Prior to assessment I will be provided with any relevant information and or available documentation regarding the individual’s circumstances and conditions. I will provide you with documentation regarding the service provided and the fee structure.

Consent   Clients are expected to consent to treatment and I will be happy to answer any questions regarding assessment and treatment procedures. Additional consent will be requested for the use of photographic/video recording of treatment sessions. All data recorded will be stored securely and will not be shared without consent.

Treatment Subsequent treatment sessions last 1 hour, of which a minimum of 50 minutes will be direct hands-on therapy, allowing 10 minutes for the discussion of goals, the writing up of treatment notes and on-site training to support therapeutic programmes. Treatment sessions are offered Monday to Friday from 9.00am to 3.00pm. Should the client be unable to meet a prearranged appointment, 24 hours’ notice is required, as other than in exceptional circumstances, a cancellation rate will apply.

Costs

·         A treatment session is defined as 1 hour of the therapist’s time. The session rate does not include any reports or equipment provided which will be invoiced separately, if required. I do not charge for travel unless the suggested travel route to the appointment is over 15 miles from my home address. Travel cost will then be charged at £0.50 per mile over that distance.

·         Any other treatment duration less than an hour would be charged at a value set by the physiotherapist depending on location and feasibility.

·         I would like to take this opportunity to advise you of the cancellation policy. Physio Home Care requires 24 hours’ notice of any cancellation.  If less than 24 hours’ notice is given, please note that a cancellation session rate will apply. Please support me to ensure chargeable cancellations are kept to a minimum by ensuring all staff and family involved are aware of their responsibilities to communicate cancellations as soon as practical.

Invoices Invoicing is on a month end basis and the payment terms are 7 days.

Payment failure Should an agreed date of payment be exceeded, treatment will cease immediately. Treatment will not be reinstated until outstanding payment is received.
